Q: What should I send to Beverly Hills Dental Studio with each case?

A: With each case, remember to include the following:
Two clear, full arch impressions (if in doubt take another one to prevent having to cut off temporaries if an area of the impression is not readable)

Photograph of the prepared teeth with dentin (stump)
shade tab in place to show color variations

Full face photograph of the patient with provisionals in place

Study model and/or detailed description of
significant changes that have to be made

Bite registration

Stick bite registration and photo of the stick bite in place

Q: How will I be billed?

A: An invoice will be delivered with each case. Also, we supply a monthly statement, which itemizes all of the month’s invoices. The statement is due upon receipt, and we accept check and all major credit cards.
